VACCINATION OF PATIENTS WITH MALIGNA NT ASTROCYTOMA WITH AUTOLOGOUS WHOLE CELL VACCINE AND GM-CS F Sloan A, Zamorano L, Diaz F, Baynes R, Dancey R, Hamm C, Wood G; Neuro-Oncology Program, Karmanos Cancer Institute of Wayne State University, Detroit , MI The presence of tumor antigens and the ability to induce an immune response against them is a pre-requisite for immunotherapy. Astrocytomas lack readily identifiable tumor antigens, have little innate immunogenicity, evade immune surveillance, and are actively immunosuppressive. Preclinical studies in rodent models established that combined active and adoptive immunotherapy could eradicate intracranial malignant astrocytoma. However, the results of human immunotherapy trials have been disappointing. The current study was designed to determine whether a tumor specific immune response could be induced in patients with recurrent malignant astrocytomas by vaccinating them with irradiated autologous tumor cells and GMCSF. Patients n 20; M F; median age 45 ; had failed external beam radiotherapy 95% ; , chemotherapy with 15 agents 85% ; , stereotactic radiosurgery 20% ; , and brachytherapy 25% ; . All were vaccinated twice by intradermal injection of 5 3 106 irradiated autologous whole tumor cells and 125 g GM-CSF into each of four skin sites, two weeks apart. The patients were tested for delayed-type hypersensitivity DTH ; at a distant site after each vaccination. No reactions were observed at the original site, demonstrating lack of an innate anti-tumor response. However, all patients developed DTH responses against autologous cancer cell associated antigens two weeks after vaccination. There was no response to GM-CSF alone, tissue culture medium, or autologous skin fibroblasts used as negative control. Toxicity was minimal. These findings demonstrate that human astrocytomas are immunogenic, and that vaccination with autologous whole tumor cells combined with GM-CSF induces a cell mediated immune response against autologous tumor associated antigens despite numerous courses of prior radiation and chemotherapy and the presence of advanced disease, for example, penicillin g acylase.
