Dr. G.B. John Mancini recently completed his two terms as the Eric W. Hamber Professor and Head of Medicine at the University of British Columbia and Head of the Department of Medicine at Vancouver Hospital and Health Sciences Centre, Vancouver, British Columbia. He is currently directing an imaging-based research program that includes facilities for quantitative coronary angiography, and ultrasound analysis of coronaries, carotids and the brachial artery for atheroma and endothelial function assessments. Dr. Mancini is a staff physician in the Laurel Cardiology Outpatient Clinic and the St. Paul's Hospital Lipid Clinic Healthy Heart Program. He also supports the Atherosclerosis Reversal Clinic at Providence Health Care. Dr. Mancini received his MD degree from the University of Toronto, completed his residency at Toronto General Hospital and was a Cardiology and Research Fellow at the University of California at San Diego. After a year in the rank of Clinical Assistant Professor at U.C. San Diego, he joined the faculty of the University of Michigan, where he became the Chief of the V.A. Section of Cardiology in 1987. He is a Fellow of the American College of Cardiology and of the Circulation Council of the American Heart Association; he is also a member of numerous other professional associations in the United States and Canada. Dr. Mancini is the President of the Canadian Society for Clinical Investigation. Dr. Mancini has published approximately two hundred and fifty journal articles, abstracts, books and book chapters. The presence of these three alleles had a positive predictive value for abacavir hypersensitivity of 100% and a negative predictive value of 97% Table 1 ; . However, this study was performed in a predominantly Caucasian population, and further studies in abacavir exposed cohorts from a range of ethnic and racial populations are required to confirm these findings in order to assess the predictive value of this genetic test for this syndrome. Also, with a lower observed incidence of abacavir hypersensitivity reactions in individuals of black race, other HLA types may be associated with this reaction. Nevirapine, a NNRTI, is also associated with a hypersensitivity reaction. This commonly involves fever, hepatitis and rash [23] with rare fatal outcomes. Lower pretreatment CD4 + lymphocyte counts are associated with a reduced rate of these reactions. Again in Western Australia, a cohort of HIV-1 positive individuals exposed to nevirapine in whom hypersensitivity reactions did or did not develop after at least six weeks of therapy were included in the analysis. Fourteen individuals developed a hepatotoxic or multi-system reaction out of 209 individuals. This group had a significantly higher CD4 + lymphocyte percentage odds ratio 5.5, p 0.003 ; and percentage carriage of the HLADRB1 * 0101 haplotype odds ratio 4.8, p 0.01 ; . In this project, healthy volunteers played a slot machine simulation and were conditioned to expect either the possibility of a monetary reward or no possibility of monetary reward for each "pull" of the slot machine. Transcranial magnetic stimulation was then delivered to record the activity of the motor cortex during conditions of both expectation and no expectation. If there exists a dopamine signal to the motor cortex that is reward-mediated, we hypothesized that the motor cortex would be inhibited during reward expectation. Currently, our data indicate that the motor cortex is more inhibited in male volunteers during reward expectation than it is during absence of reward expectation corrected values 0.76 vs. 0.82, p 0.02 ; , although female volunteers show no significant difference. Accomplishments The Wits CTU, in general and the Coronation CRS in particular, have extensive experience in the design, implementation and analysis of pediatric HIV treatment and PMTCT research, including clinical trials as well as providing compassionate care to the community. The national PMTCT program has been dogged by poor infant testing rates throughout the country. This is in large part due to the lack of a formal follow up plan and especially the lack of an early HIV diagnostic protocol. Our own experience from a study done in 2002 at Coronation CRS showed a poor follow up rate at 12 months of age, despite the existence of a dedicated service for this Sherman, 2004 ; . Against this background, Coronation CRS adopted a more efficient early diagnostic protocol using PCR as soon as the Department of Health approved it in 2004. The result has seen an improved rate of diagnosing infants and thereby yielding more asymptomatic infected children for further work-up and follow-up. Our own research on 300 exposed infants has contributed significantly to the revised protocol Sherman, 2005 ; . This has also resulted in several more children beginning ARV treatment prior to becoming symptomatic or dying. Data to this effect was presented at the South African AIDS Conference in Durban Technau, 2005 ; . This Infant Follow-up is arguably the largest cohort of such an infant follow up within the public service outside of research. Several studies are underway at Coronation see Table 1 below ; that are looking at this specific group of infants.
