Cles Fig. 5B ; . On the other hand, difference in the yield between two and three extraction cycles was only 5%. That is why we decided to use two extraction cycles see Fig. 5B ; . 3.4.3. Effect of solvents Analytical signals of the two prevailing isoflavones daidzin and genistin ; were chosen for the comparison of different pre-selected solvents, i.e. acetonitrile, ethanol and methanol Fig. 6 ; , the results for the former being expressed as 100%. The extraction yields obtained for the extraction of daidzin and genistin with acetonitrile were about 6080% of the yields of the extraction with methanol. The influence of the acetonitrile content in the extraction agent is shown in the inset of Fig. 6A. The extraction efficiency rapidly decreased with the increasing content of acetonitrile in the case of daidzin. The extraction yields of about 20% were obtained for the ethanolic extracts see the inset of Fig. 6A ; . Methanol was selected as the most suitable solvent for ASE extraction for this kind of the soy food samples Fig. 6A ; . Moreover, peak areas of daidzin and genistin linearly decreased with the decreasing content of methanol in the extraction agent. 3.4.4. Effect of sample amounts, pressure and temperature The decreasing yield of the ASE extraction with the increasing amounts of sample see Fig. 6B ; could be explained by the thicker layer of the sample in the extraction cell. The effect of pressure was measured in 90% aqueous methanol. It is evident from the results presented in inset of Fig. 6B that the lower pressure decreased yield about 20%. The main reason why the high pressure is used during the extraction.

Because of the potential for adverse effects on the nursing infant, a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother. The importance of the way in which information is presented is emphasised by Fahey et al [8], who gave 182 health authority members results from a randomised trial on breast cancer screening and results from a systematic review on cardiac rehabilitation. The results were presented to them Table 5 ; in four different ways and vasodilan, for instance, soy isoflavones benefits. Modes for this new modality. Concern and caution are clearly indicated. In developing a comprehensive QA system, it is useful to separate the complete treatment process into three sequential elements: a Dose and MU calculation; b information transfer from planning system to R V system to delivery system; and c dose delivery. Each step in the process has its own potential sources of error, and the physicist should develop checks for each. These checks will involve some combination of inspection, calculation, and experiment. In the following sections we describe some of the possible techniques that have been used; each has its strengths and limitations. Each physicist and facility will need to balance patient-specific tests, such as described in this section, with standardized MLC and linac performance tests, such as described in Sec. II A. Patient-specific verification measurements test many but not all aspects of planning and delivery in a combined fashion. Patient-specific calculations combined with frequent machine QA represent another approach. The latter is the norm for conventional treatments and may become so for IMRT as the field evolves. In these early stages, physicists need to carefully assess the overall structure of their QA tests and frequencies. 264s-264s 1 ; publisher: elsevier previous article next article view table of contents key: - free content - new content - subscribed content - free trial content language: english document type: abstract doi: 1 1016 s0924-9338 99 ; 80475-1 this article is hosted on another website and ketorolac.

Medicaid--which accounted for over $2.2 billion worth of pharmaceutical manufacturer rebates in 1997--is not alone in its use of rebates. As the number of HMOs and PBMs increased, manufacturers began to offer rebates in an effort to gain access to managed care patients.26 These programs add another dimension to drug pricing variability. The price a manufacturer sets often changes as it makes its way through the distribution chain and onto the negotiating table. It is not unusual for one specific product to be priced differently in different markets see Table 4 ; . Many cross-national drug pricing comparisons have been made over the years. While these findings are significant, they merit caution in how the comparisons are made and in how the conclusions are drawn. Because markets, demographics, and values vary, and because medical practices and economic circumstances also vary, it is difficult simply to transfer one country's pricing methods to another. Nevertheless, for many reasons, price differentials between products purchased in the U.S. and other countries are oftentimes substantial, leaving many to question why the gap is so wide as well as whether and how it should be narrowed.

Facts: Narcotics officers received information from a reliable informant that defendant was engaged in drug trafficking and home invasion robberies. An undercover officer, pretending to work for a major drug dealer, contacted Smith and suggested that it would be cool if she would steal a shipment of cocaine from the officer's supposed boss. Smith readily agreed, indicating that she was a professional at this and had done this kind of thing before, using her own crew. The police therefore set up a sting by getting a court order for the release of 85 kilograms of cocaine from a supply that was scheduled for destruction. They then obtained a vacant house and a van. They put the 85 kilograms of cocaine in the van and parked it in the garage of the house, leaving the key in the van's ignition. In the house, a table was set up with a money counter, pay-and-owe sheets, rubber bands for wrapping money, and other paraphernalia associated with drug trafficking. A key to the front door was put under the doormat. A SWAT team was positioned around the house. With this all set up, a call was then made to the defendant giving her directions to the house, where to find the door key, and where the cocaine was located. Smith and her cohorts arrived at the house. While Smith waited in the car, the others, one of whom was armed with a firearm, entered the house. Finding the place empty, they went to the garage and started to back the van out of the driveway when the police activated a kill switch. Everyone was arrested at the scene. Smith and her codefendants were convicted of a number of charges including the theft of the cocaine and the van and the attempted transportation of cocaine. Defendant Smith received a 36year prison sentence, with her codefendants receiving 47 and 33 years, respectively. 25 years of each of their sentences were from a sentencing enhancement pursuant to H&S 11370.4 a ; 6 ; , for attempting to transport over 80 kilograms of cocaine. Defendants all appealed. Held: The Second District Court of Appeal Div. 7 ; affirmed all but the sentencing enhancement. 1 ; As to the theft of the cocaine and the van, defendants argued that because these items were taken with the victims' i.e., the police ; consent, there could be no theft. Or, at the most, it was only an attempted theft. The Court, however, noted that any consent involved was a consent to take the property temporarily. Defendants, in moving the cocaine and van, had the intent to permanently deprive the victims of their property. The police never consented to the permanent taking of the property. Therefore, the taking was "trespassory, " and complete. This Court further rejected the defendants' argument that such a sting operation could only be, at most, an attempted theft, finding that there was no such authority to the effect that the crime could be no more than an attempt. Defendants moved the property with the intent to permanently deprive the victims of their property. That is a completed theft. 2 ; The Court also rejected defendants' argument that they had been "entrapped." The test for entrapment in California is whether the conduct of the law enforcement officers is likely to induce a normally law-abiding person to commit the offense. The fact that it was the police who came to the defendants with the plan, scripted the entire scenario, represented the results as very lucrative, and made it as easy as possible, does not make it entrapment. A normally law-abiding person still would still not have been interested in stealing drugs from drug traffickers. Also, the police actions in this case did not constitute "outrageous police conduct; " a "due process" issue. The police came to Smith based upon and ketotifen. Partnership Dr. E. Measurements were carried out in duplicates by a calibrated blood gas analyzer. RESULTS: Pleural fluid pH values were significantly p 0.05 ; lower when collected in heparinized versus non-heparinized syringes at all time intervals T0: pH H ; 7.37 0.10 vs. pH NH ; 7.39 0.10, T1: pH H ; 7.37 0.11 vs. pH NH ; 7.39 0.11, T2: pH H ; 7.37 0.12 vs. pH NH ; 7.38 0.11 ; . With heparinized syringes, there was a significant p 0.05 ; decrease in pleural pH values only at T2 versus T1. There were no significant changes in pleural pH values over time with the nonheparinized syringes. CONCLUSION: There were statistical differences in the values of pleural pH when using heparinized versus non-heparinized syringes. In heparinized syringes, the pleural pH values decreased at two hours but not at one hour post collection, therefore it is advisable to process pleural fluid for pH measurements no later than one hour of collection when using heparinized syringes. CLINICAL IMPLICATIONS: for serial pleural pH measurements the same type of syringes either heparinized or non-heparinized ; should be consistently used and samples collected in heparinized syringes can be processed up till one hour of collection. Almeida PharmD Milena M. Acencio BS Fabricio D. Gomes BS Roberta K. Sales MD Marcia Seiscento MD Lisete R. Teixeira MD Pulmonary Division, Heart Institute InCor ; and Department of Pathology of Univer, Sao Paulo, Brazil PURPOSE: To evaluate the interference of pre-analytic factors as storage time and temperature in pleural fluid adenosine deaminase ADA ; measurement in pleural effusions. METHODS: 27 pleural effusions obtained from patients with neoplasm 20 ; or tuberculosis 07 ; were analyzed. The pleural effusions were collected into tubes containing EDTA. The ADA determination was realized 1, 3, 7, and 28 days after the toracocentesis and the samples were stored at 4C refrigerator ; or -20C freezer ; . The ADA determined immediately after the pleural collection T1 ; was used as gold standart to the comparison with the results obtained from the other times. The ADA activity was determined by the modified Giusti method. Statistical analysis: The Wilcoxon test was used to compare T1 with the resuts obtained at 1, 3, 7, and 28 days, for both temperatures. We used the Pearson coefficient to evaluate the correlation between T1 and the other times evaluated. RESULTS: Pleural ADA from T1 was 36.45 U L 23.2 55.1 ; . We do not observe differences statistically significant between this measurement with those obtained from 1, 3, 7, and 28 days, for both temperatures 4C or -20C ; . The correlations between T1 and the other times were superior to 0.90 until the tenth day for both temperatures, with a tendency of decreasing around the 28th for the temperature of 4C r 0.86 ; or -20C r 0.83 ; . CONCLUSION: ADA levels are stable for at least 28 days in samples of pleural fluids if they were maintained refrigerated 4C ; or freezed 20C ; . CLINICAL IMPLICATIONS: Considering that samples of pleural effusion can be sended to ADA determination many hours after the toracocentesis or that frequently we have to confirm ADA results, we consider safe to quantify this enzyme until at least 28 days after the collection, if the samples were maintained at refrigerator or freezer. Subjects were invited to the metabolic unit on 6 occasions t0, t4, and t8 on each intervention arm ; , after 12-h overnight fasts. To exclude potential second-meal effects, a set low-fat evening meal 10 g fat ; was consumed the evening before each of the study days. Recipes for the preparation of the meals were provided to the participants. Energy contents of the meals were comparable. Subjects were asked to consume two fruit cereal bars d Health & Diet Food, Manchester, UK ; , one in the morning and one in the afternoon, in addition to their normal diet. During the treatment period, cereal bars were enriched with 2 25 mg isoflavones d, with a genistein to daidzein ratio of 2: 1 "Solgen 40", Solbar Plant Extracts, Ashdod, Israel ; . Thus, isoflavpne intake in the treatment group of the present study was in the upper range of the daily isflavone intake in traditional Asian diets 15 50 mg d ; [28]. The product was tested before packaging and during the study by HPLC, to ensure stability of the isoflavones [29]. Placebo did not contain any isoflavones. Each cereal bar 40 g ; had an average nutrient content of energy 652 kJ protein 2.6 g; carbohydrate 17.3 g; fat 8.5 g; fiber 1.8 g; sodium 0.012 g. Subjects perceived the isoflavoneenriched and placebo cereal bars as identical in taste and visual appearance. Habitual diet was assessed by estimated 3-d food records three times during the study. Diet diaries were completed at baseline t0 ; and after 4 weeks t4 ; of each intervention arm. All food records included two week days and one weekend day. Nutrient intake was calculated based on the German Food and Nutrient Data Base Bundeslebensmittelschlssel BLS II.3 [30]. To avoid weight gain, subjects were advised to replace snacks with the cereal bars. Subjects kept daily records of cereal bar consumption and well-being in a study diary. Dietary compliance was further assessed by measurement of phytoestrogen concentrations in 24-hour urine [31], which was collected at start and end of each intervention period. Body weight was measured at each visit. Biochemical parameters Blood was collected in ice-chilled EDTA tubes for the analysis of glucose, ghrelin, and PYY. Establishing the presence of a significant amount of bacteria 108 bacteria L ; in a suitable urine specimen is often helpful for presumptive diagnosis. In most cases, adequate specimens are obtained by collecting mid-stream clean-voided urine. Suprapubic aspiration collection of urine sample using a suction device ; , the gold standard, or catheterization may be necessary for patients unable to provide a proper sample. A polymicrobial result usually suggests contamination. Quantification of bacteria can be achieved by microscopic examination. This step also allows for detecting the presence of pyuria, hematuria, and casts and for preliminary microorganism identification. Pyuria is defined as a white blood cell WBC ; count greater than 10 cells mm3. This finding and hematuria are non-specific and patients with pyuria or hematuria do not necessarily have an infection.27, 28 Presence of white cell casts is strong evidence for pyelonephritis but can also be found in other renal diseases. Biochemical tests can assist in excluding infection. Bacteria reduce nitrate usually found in urine and their presence is revealed by detecting the degradation product nitrite. The detection of leukocyte esterase, an enzyme found in neutrophils, correlates with 10 WBC mm3.29 Despite good sensitivity, specificity, and negative predictive value, the leukocyte esterase test may not always be reliable in daily practice see Table 3 ; .30 If negative, pyuria must be ruled out by microscopic examination. Cognitive function declines after menopause. Estrogen replacement therapy may improve cognitive function but is associated with serious side effects. A possible alternative, phytoestrogens, was effective in improving memory in college students when supplemented for a limited time. The effect of long-term low dose exposure to phytoestrogens as is achieved through the habitual diet is largely unknown. We aimed to study the effect of relatively high dietary intake of isoflavones and lignans within a Western population on cognitive function. We conducted a cross-sectional study in 301 Dutch women aged 60 75 y. Dietary isoflavone and lignan intake was assessed using a food frequency questionnaire covering the habitual diet the year preceding enrollment. Endpoints were memory Rey Auditory Verbal Learning Test, immediate recall, delayed recall and recognition, Digit Span forward and reversed, and the Doors test ; , complex attention tasks Digit Symbol Substitution, and Trailmaking A1, A2 and B ; , and verbal skills Verbal Fluency A and N, animals and occupations, and the Boston Naming Task ; . Radiologic features of Castleman's disease occupying the renal sinus Nishie A., Yoshimitsu K, Irie H, Aibe H, Tajima T, Shinozaki K, Nakayama T, Kakihara D, Naito S, Ono M, Muranaka T, Honda H From the Department of Clinical Radiology and Urology, Graduate School of Medical Sciences, Kyushu University, Maidashi, Higashi-ku Fukuoka, Department of Radiology Kitakyushu Municipal Medical Center, Kokurakita-ku Kitakyushu and National Kyusyu Medical Center, Chuo-ku Fukuoka, Japan AJR J Roentgenol. 2003; 181: 1037-40 Objective: Our purpose was to describe the radiologic findings in five abnormalities in three patients with Castleman's disease occupying the renal sinus. Conclusion: Common findings such as mild homogeneous enhancement passing through the mass of the collecting system with mild hydronephrosis on contrast-enhanced CT and hypointense signal on T2-weighted images were obtained. Castleman's disease may be considered in a differential diagnosis of a mass occupying the renal sinus, although it is difficult to differentiate from malignant lymphoma. Abstract Edited Purpose: To describe the radiologic findings in five abnormalities in three patients with Castleman's disease occupying the renal sinus.

M-PM-Posl29 CIRCULAR DICHROISM AND INFRARED OF CYTOCHROME OXIDASE AND OF Ca + 2-ATPase ORIENTED IN MULTILAMELLAR FILMS. M.D. Bazzi and R.W. Woody, Dept. of Biochem., CSU, Ft. Collins, CO 80523. Oriented multilamellar films of purified cytochrome oxidase CCO ; and of Ca + 2-ATPase have been obtained by the isopotential spin-dry method Clark, Biophys. J. 31, 65, 1980 ; . The orientation of the secondary structure of these proteins has been examined by circular dichroism CD ; and polarized infrared IR ; . In the CD spectrum of CCO films, the 208nm band observed in the solution spectrum is greatly diminished, while the 222nm and 195nm bands are reduced in intensity, with the latter shifted to 198nm. In the Ca + 2-ATPase film spectrum the 208nm band is almost completely eliminated and the 192nm band is reduced in magnitude. The solution and the film spectra have been deconvoluted into gaussian components. This analysis indicates that the helix axes are tilted by 35 deg. in CCO and 25 deg. in Ca + 2-ATPase, with respect to the normal. The CD spectra also indicates that both proteins contain some 8-sheet. IR linear dichroism has been measured with the plane of the film normal or tilted by 60 deg. with respect to the incident beam. The amide I of CCO is increased while amide II shows little change in the tilted position in comparison with the normal position. In the case of Ca + 2-ATPase films, amide I is increased while amide II is decreased in the tilted position. These results indicate that the helices of CCO and Ca + 2-ATPase predominantly oriented normal to the plane of the film. CCO films show strong amide V band 700 cm-1 and weaker amide V bands at 650 and 600 cmn1. These bands attributable 8-sheet, random and a-helical structures, respectively. The amide I of Ca-ATPase has two shoulders at 1690 cm7l and 1630 cm-1 indicating the presence of 8-structure in Ca + 2-ATPase. Supported by USPHS grant, GM22994.
