Rajiv Singh, Brian Pentland, John Hunter, Frances Provan, Astley Ainslie Hospital, Edinburgh. Researchers from Astley Ainslie Hospital, Edinburgh, reported the results of a study of 154 people with PD who were referred to the centre's driving assessment centre. The multidisciplinary team used a combination of clinical tests, reaction times on a test rig and an in-car driving test to assess whether PD patients were suitable to drive. The team considered that 66% of the PD patients were fit to continue driving, although 46 required automatic cars and 10 needed car modifications. Several factors were correlated with unsuitability to drive: increasing disease severity; advancing age; other medical conditions, particularly dementia; disease duration; reaction time on test rig; and the score on driving test. Drug treatment and length of driving history did not predict whether a patient would be unsuitable to drive. Severe disease, slow reaction time, moderate disease associated with another medical condition and the score on car testing emerged as showing the strongest correlation with driving ability. In conclusion, the authors noted that most PD patients are safe to drive, although many benefit from car modifications or using an automatic vehicle. "A combination of clinical tests and in-car driving assessment will establish safety to drive and a number of clinical correlates predict likely outcome, " they added.
Extensively studied compounds with robust effects for bipolar depression. Standard antidepressants now represent one of only several pharmacologic strategies for the treatment of bipolar depression, and determining the clinical indications for their use relative to alternative antidepressant approaches likely should be a tailored decision based on the profile of clinical features, past treatment outcomes, and risks unique to an individual patient, for example, cloxacillin mechanism.

Description of the studies Gough and colleagues 1997 ; 100 The double-blind RCT by Gough and colleagues compared a subcutaneous injection of G-CSF with a placebo saline solution ; in 40 inpatients with a DFU with extensive cellulitis the majority occurring in the forefoot ; over a 7-day period.100 G-CSF dosage was initially 5 g kg day daily for 2 days ; , and was then titrated against the patient's neutrophil count, reduced to 2.5 g kg day daily for 2 days ; , and then given on alternate days up to 7 days ; . All patients received an intravenous combination of four antibiotics ceftazidime, amoxycillin, flucloxacillin and metronidazole ; , appropriate glycaemic control, foam dressings and podiatric treatment. Two patients in the placebo group underwent toe amputation compared with none in the G-CSF group [RR for amputation 5 Haldane approximation ; , 95% CI 0.3 to 98]. Two patients in the placebo group required extensive debridement under anaesthesia, compared with none in the G-CSF group [RR for debridement 5 Haldane approximation ; , 95% CI 0.3 to 98]. The study is underpowered to detect clinically important differences in debridement or amputation outcomes as statistically significant. The median time to resolution of cellulitis was 7 days in the G-CSF group range 520 ; and 12 days in the placebo group range 593 ; . The median time to negative swab was 4 days in the G-CSF group range 410 ; versus 8 days in the placebo group range 593 ; , and although there were insufficient data available to allow us to calculate the CIs for the difference in time, the authors stated that the difference was statistically and cromolyn.

The agreement provides that the Company will exchange its 7% xed-rate payment obligation for variable rate payments of six-month LIBOR plus 2.409%. While the objective of the swap is to reduce the Company's overall interest cost, if LIBOR rates increase over 4.59%, our cost of debt will equal or exceed the current xed rate amount subject to the swap. The swaps have terms expiring at the maturity of the debt. The swap arrangements are with dierent counterparties than the holders of the underlying debt. RESIDENT BUSH'S national energ y strategy is a prescription for environmental disaster that would leav e America as dependent as ever on unstabl e supplies of foreign oil . What should have been a proposal tha t arrived dead on Congress' doorstep, is aliv e and kicking, thanks to an elaborate life-support system provided by the deep-pockete d oil, auto and nuclear lobbies and their allies i n Congress . The Bush plan has been embodied in th e form of the misnamed National Energy Secu rity Act of 1991 the Johnston-Wallop bill, S 1220 ; , championed by oil patch Senato r Bennett Johnson of Louisiana, chair of th e Energy & Natural Resources Committee , and Senator Malcolm Wallop of Wyoming . Senator Johnson is using his considerabl e powers to ram the Johnson-Wallop bil l through the Senate process as quickly a s possible . Such hurried decision making is disturb ing in its own right ; more unsettling is the fac t that a national energy policy that the Ameri can people clearly don ' t want is makin g headway with some elected officials . Pollin g data from January of this year shows that three of every four Americans want an energy policy based on energy efficiency, con servation and renewable energy sources , rather than business as usual . And the Johnston-Wallop bill is unmistakably business as usual . Instead of encour aging energy conservation and efficiency, i t champions drilling in the nation ' s last remaining arctic wilderness--the Alaska Arcti c National Wildlife Refuge--and along its pris tine and vulnerable coasts . At a time when oi l companies are reporting record profits from a war that irrefutably demonstrated the huma n and environmental costs of petroleum depen dence, it would be a tragedy to hand ove r America's few remaining natural jewels t o the oil giants.

Zones were larger 5.4 - 6.2 mm ; . The effectiveness and sensitivity of the BsDA on the following days of the experiment, at low antibiotic residue concentrations in milk, was also more satisfied than in B. subtilis pH 6.0 ; plates. The other plates with B. subtilis pH 7.2 and 8.0 ; and Micrococcus luteus showed very low sensitivity and the means of zones were lower than in previous two tests Table 1 ; . The last administration of Ampiclox was 48 h after beginning of the experiment and the last positive result with B. subtilis pH 6.0 ; was recorded 24 h and using BsDA 36 h after the last antibiotic administration. Results of the antibiotic residue detection methods after Ampiclox L.C. infusions in combination with single injection of Norocillin L.A. are shown in Table 2. The results recorded in B. subtilis pH 6.0 ; plates, in comparison with BsDA 12 and 24 h after the administration of the antibiotic, were similar to those after the single administration of Ampiclox. The last positive results with B. subtilis and BsDA were recorded 24 h after the last antibiotic administration. Results of Delvotest SP obtained from the examination of milk samples from both groups after mastitis treatment show that these samples on day zero were free of antibiotic contamination. The milk samples were examined every 12 h. Data of group 1 demonstrated positive results of Delvotest SP 48 h after the last drug administration. In group 2, there were positive results in all dairy cows at the 36th h and 3 from them had indecisive results after 48 h. Using HPLC, the highest mean concentrations of penicillin 2693.5 g -1 ; and amoxicillin 2063.0 g -1 ; in milk from group 1 were detected 12 h after the second administration and cloxacillin 22383.25 g -1 ; 12 h after the last administration. In group 2, the highest mean concentrations of amoxicillin 2127.4 g -1 ; and cloxacillin 21697.4 g -1 ; were detected at hour 12 after the last antibiotic administration. Afterwards, the mean concentrations gradually decreased and the residues were still present in milk groups 1 and 2 ; 48 h after the last administration. In both groups, no residues were detected after 60 h.

Maintenance of sinus rhythm with antiarrhythmic drugs does not confer a mortality advantage and does not reduce the thromboembolic risk compared with rate control. These medications remain important therapeutic options for certain patients, including those with symptoms not mitigated by rate control. The appropriate choice of antiarrhythmic medication made with attention to patient clinical characteristics and potential drug toxicity reduces the risk associated with these agents. New approaches to drug development and screening for risk for adverse events may ultimately reduce the toxicity of these agents. Rapid advances in pacing and ablation technologies either alone or in combination with antiarrhythmic drugs will also provide new options for maintenance of sinus rhythm.
