One of the great ironies of the present-day industrialized world is serious disease and death brought about by too much rich food and too little physical exertion. The incidence of obesity has increased to the point that one in two American adults is now considered overweight 1 ; . Pathologies linked to obesity, such as type 2 diabetes, hypertension, and cardiovascular disorders, are also increasingly prevalent in our society. The tight linkage of obesity, insulin resistance and frank diabetes ; , dyslipidemia, and hypertension has been widely observed and has been dignified with a label syndrome X, or the metabolic syndrome 2 ; . The exact pathogenic relationships between the component conditions of the metabolic syndrome are complex and incompletely understood, despite significant and ongoing efforts to identify susceptibility genes in human populations and animal models. The convergence of these conditions in the metabolic syndrome is not an area isolated to mere academic interest: Coronary and peripheral vascular disease leading to myocardial infarction and stroke is the unhappy fate of many affected individuals. In an ideal world, the metabolic syndrome would be treated by diet and exercise, leading to weight loss. Even relatively modest degrees of weight loss have been shown to improve markers of the metabolic syndrome, such as blood pressure, serum cholesterol, and insulin levels. Unfortunately, most patients find the necessary dietary and exercise regimens to be difficult. Even if the difficulty is surmounted, they find that their bodies resist any deviation from the "setpoint" of their elevated weight. Objectives To assess the evidence for the existence and prevalence of sleep attacks in patients taking dopamine agonists for Parkinson's disease, the type of drugs implicated, and strategies for prevention and treatment. Design Review of publications between July 1999 and May 2001 in which sleep attacks or narcoleptic-like attacks were discussed in patients with Parkinson's disease. Results 124 patients with sleep events were found in 20 publications. Overall, 6.6% of patients taking dopamine agonists who attended movement disorder centres had sleep events. Men were over-represented. Sleep events occurred at both high and low doses of the drugs, with different durations of treatment 0-20 years ; , and with or without preceding signs of tiredness. Sleep attacks are a class effect, having been found in patients taking the following dopamine agonists: levodopa monotherapy in 8 patients ; , ergot agonists apomorphine in 2 patients, bromocriptine in 13, cabergoline in 1, lisuride or piribedil in 23, pergolide in 5, ; and non-ergot agonists pramipexole in 32, ropinirole in 38 ; . Reports suggest two distinct types of events: those of sudden onset without warning and those of slow onset with prodrome drowsiness. Conclusion Insufficient data are available to provide effective guidelines for prevention and treatment of sleep events in patients taking dopamine agonists for Parkinson's disease. Prospective population based studies are needed to provide this information and carbidopa.
The development of surfactants based on carbohydrates and oils is the result of a product concept that is based on the exclusive use of renewable resources. In industry, saccharose, glucose, and sorbitol, which are available in large amounts and at attractive prices, are used as the preferred starting raw materials. The selective functionalization of saccharose and sorbitol with fatty acids for the construction of a perfect amphiphilic structure cannot be realized in simple technical processes because of the polyfunctionality of the molecule. This is why the products offered on the market contain different amounts of mono-, di- and tri-esters and are therefore only suitable for particular applications, for example, as emulsifiers for foodstuffs and cosmetics or, in the case of the sorbitan esters, also in technical branches such as in emulsion polymerization. The ideal raw material for selective derivatization is glucose. Reaction with fatty alcohol produces alkyl glucosides; N-methylglucamides are prepared by reductive amination with methylamine and subsequent acylation. Both products have proved to be highly effective surfactants in washing and cleansing agents. The alkyl glucosides have also additionally established themselves in the cosmetic products sector, as auxiliaries in crop protection formulations and as surfactants in industrial cleansing agents and today can already be said to be the most important sugar surfactants based on the yearly production amounts. Abortions were not observed in any of the control vixens, although they were observed in three of four vixens in both groups that received the 2 50 g kg1 and 100 g kg1 doses of cabergoline at day 28 of pregnancy Table 2 ; . Emesis was observed in the single vixen treated with 100 g cabergoline kg1 at day 28 that was not observed to abort and, therefore, it is likely that a full dose of cabergoline was not ingested. Notably, no abortions were observed at the same dose at day 35 and only one occurred in the group treated with 100 g cabergoline kg1 at day 42 Table 2 ; . Probit and logistic regression models indicated significant effects P 0.05 ; of dose and timing of cabergoline on the probability of abortion and little difference in the goodness of fit for either model. The probability of abortion increased with dose, but decreased when treatment occurred later in pregnancy. Lactation commenced in all vixens that whelped and had received a single or double dose of cabergoline, irrespective of cub survival. Copious milk could be expressed from the teats of all vixens that lost their litters, although comparative lactation performance between vixens treated with cabergoline and control animals could not be estimated. Four of five vixens that received 100 g cabergoline kg1 each day from day 42 to day 46 of pregnancy aborted their litters, whereas one vixen whelped five cubs that survived to weaning. One vixen in the control group failed to whelp, although no indication of an abortion was detected. Means of 0.8 SD 1.8 ; and 4.4 SD 1.9 ; cubs per litter were born in the cabergoline-treated group and the control group, respectively. The central nervous system has well-established effects on the heart. A hierarchy of autonomic control is recognised involving cortical levels of modulation. It is therefore perhaps not surprising that epilepsy can result in changes of cardiovascular physiology. However, the precise dynamics are not well understood and need to be elucidated in view of their potential role in sudden unexpected death in epileptic patients SUDEP ; . Surges of sympathetic outflow have been postulated to occur during seizures, which may then contribute, to the pathophysiology of SUDEP. Temporal lobe epilepsy is particularly troublesome in causing changes in autonomic activity and case reports of ictal associated tachycardia or bradycardias are frequently documented. Interestingly interictal changes of sympathetic cardiovascular tone have also been demonstrated. To address this relationship further, Hilz and colleagues have studied autonomic parameters before and after surgery in 18 TLE patients. Variability of heart rate and blood pressure were determined power spectral analysis ; incorporating changes attributable to respiration. Each signal had a combination of high and low frequency analysis. Calculation of baroreceptor sensitivity was also performed derived from the relationship between these parameters. The standard measures of cardiovascular function did not change. Low frequency components of HR and BP showed an average reduction of over 40% following surgery. Baroreceptor sensitivity also changed. This supports the conclusion that sympathetic tone is augmented in TLE patients. Phonophobia [23]. It affects 5 - 20% of the general population [24, 25]. Tension-type headache is characterized by recurrent episodes of headache lasting minutes to days, with pain typically pressing tightening in quality, of mild to moderate intensity, bilateral in location. Cluster headache is characterized by attacks of severe strictly unilateral pain, orbitally, supraorbitally and or temporally located, lasting 15-180 minutes and occurring from once every other day to 8 times a day. Attacks occur in series lasting for weeks or months. Cluster headache is rare, occurring in less than 1% of the population [27]. Primary headaches are chronic disorders, recurring for many years, mainly in young and adult people [28]. Moreover, despite recent advances, the pathophysiology of primary headaches remains incompletely understood [29], and there are no causal and definitive therapies for these disorders [30, 31]. Secondary or symptomatic headaches constitute a minority and may be a symptom of a wide variety of conditions that adversely affect the brain: neoplastic, infectious, toxic or metabolic. Some are not serious e.g., headache associated with fever ; , but others are life threatening, such as subarachnoid haemorrhage [32]. If quickly diagnosed, they can be treated and, in many cases, definitively cured. Doctors, are you utilising your practice nurses to the best of their ability? Are you aware of the opportunities being offered by the nursing in general practice program? One of the achievements of this program to date has been the pilot Pap smear Provider's course. was lucky enough to run one of the three pilot programs. The course involved North and West Qld Primary Health Care, Far North Qld Rural Division of General Practice & Townsville Division of General Practice. On completion of the programs, they will be evaluated, in preparation of a state wide delivery into 2006. Five of the nurses to undertake the training were from within the Townsville Division. The aim of the course was to provide an opportunity for registered nurses working in general practice to develop competency in cervical screening. Successful completion will allow the participant to be competent to perform reproductive health assessment and Pap smear screening. The Australian Practice Nurses Association APNA ; has supported the release of a Medicare item number for Practice Nurses performing Pap smears in general practice. The course combined both a theory component and a clinical attachment. All participants were required to undertake a 3 day theory component; for the clinical attachment, applicants had a choice of options. The practice principals of two surgeries chose to supervise their practice nurses. This required attendance at a workshop which was facilitated by Dr Danielle Haller in her Family Planning role. The workshop had a video link to Ingham and allowed interaction and discussion to flow freely between participating GPs, members of Family Planning, and representatives from the divisions and Qld Divisions of General Practice. Queensland still has the lowest screening rates in Australia, with Townsville one of the lowest in Queensland. On successful completion of the course, the nurses will be eligible to apply for authorisation as a registered nurse Pap smear provider and to access data on the Queensland Pap smear registry. It will also give the doctors the opportunity to utilise their nurses to offer the choice of this procedure being performed by either a male or female clinician and thus remove a barrier that reduces the uptake of Pap smear screening in general practice.
