| Avandia psoriasisPrevious studies on the extrapancreatic actions of sulfonylureas in cultured cells have focused on the effects of these drugs on glucose transport and metabolism Rogers et al., 1987; Muller and Wied, 1993; Tsiani et al., 1995 ; . It has been reported that sulfonylureas directly stimulate glucose metabolism and potentiate insulin actions by enhancing GLUT-1 and GLUT-4 expression, translocation, and activation Mul.

Table VII. Pharmacodynamic properties of synthetic curares4, 9. have suggested that the two groups should be called depolarizmg agents and curare competitors. We, ourselves propose to denote decamethonium and succinylcholine by the name of leptocurares, and tubocararine and gallamine by the title of pachycurares. The advantage of such a nomenclature lies in the fact that it does not prejudge in any way the mode of action of these substances. The main differences between the pharmacodynamic effects exercised by the two types of curare rest chiefly in their action on Batrachian and bird muscle. In birds the pachycurares are typically curare-like in action while the leptocurares provoke a contraction followed by curarization. In mammals the differences between the two groups of products seem less clear. There are important differences between the reactions of different species, and between those of different muscles in particular, because the reactions of various species and different muscles of the same species are not exactly coincident, and because there seems to be every gradation of action between depolarization and curare competition. The distinction between the two groups, which is only relative as regards the mechanism of action, and whose significance varies according to the neuromuscular preparation under consideration, keeps all its interest in the study of relationships between chemical structure and pharmacological activity. From the clinical point of view, the main factor in the classification of curares is the duration of action of the different compounds. In this respect, the introduction of short-acting curare-like substances and, in particular, succinylcholine is an important acquisition. The relative ease with which succinylcholine is hydrolysed by the pseudocholineesterases and the very.

There are no data available establishing that continued therapy with etanercept at a dose of 50 mg twice weekly beyond 3 months of initial therapy ; offers a significant advantage over continued therapy with 25 mg of etanercept twice weekly. The long-term safety of continuous therapy with 50 mg etanercept twice weekly in patients with psoriasis has not been established. Patients with plaque psoriasis n 409 ; who responded to etanercept after a 24-week blinded, randomized controlled trial were re-randomized to receive their previous dose 25 mg or 50 mg twice weekly ; or placebo for a single 12-week retreatment after disease relapse. Response rates seen after re-treatment were similar to those seen during the original treatment. There was no significant difference between the higher and lower doses. Patients with guttate, erythrodermic, or pustular psoriasis at the time of screening were excluded from the clinical trials and use of etanercept in these conditions is considered investigational.[12].

The 1998 Wingspread Statement on the Precautionary Principle summarizes the principle this way: "When an activity raises threats of harm to the environment or human health, precautionary measures should be taken even if some cause and effect relationships are not fully established scientifically." All statements of the Precautionary Principle contain a version of this formula: When the health of humans and the environment is at stake, it may not be necessary to wait for scientific certainty to take protective action.

Pharmaceutical turnover Total pharmaceutical turnover in 2005 was 18, 661 million compared with 17, 100 million in 2004, an increase of 8% CER. In sterling terms turnover increased 9%, principally due to the strength of the Euro and other International currencies. Pharmaceutical turnover by therapeutic area GSK's ability to continue to deliver pharmaceutical turnover growth is primarily due to an exceptionally broad product portfolio of fast-growing, high-value products. Sales of GSK's largest product, Seretide Advair, were up 22% to 3.0 billion and continued to gain market share across all regions. Market share by value in the anti-asthma and COPD therapy class was 27% in Europe and 33% in the USA, an increase of 2 percentage points in both cases compared with 2004. Sales of diabetes treatments were also strong, with Avanda Avandamet up 18% to 1.3 billion. GSK launched Avandiaa for the treatment of type 2 diabetes in 1999 and a combination product, Avandamet, for blood sugar control in 2002. The product group was expanded further in February 2006 with the launch in the USA of a fixed-dose combination treatment, Avandaryl, which combines Avanndia with a sulfonylurea. In 2005, Avanria Avandamet achieved a market share by value in oral anti-diabetics of 14% in Europe and 35% in the USA, up 3 and 6 percentage points, respectively. Other fast growing products were Lamictal for epilepsy bipolar disorder, up 24% 0.8 billion ; , Valtrex for herpes, up 21% 0.7 billion ; , Coreg for heart disease, up 32% 0.6 billion ; and vaccines, up 15% 1.4 billion ; . In addition, in 2005 there was a rapid uptake of a number of high potential products such as Requip, for restless legs syndrome sales up 34% to 156 million ; , Avodart for benign prostatic hyperplasia sales doubled to 129 million ; and Boniva Bonviva for the treatment of osteoporosis, which was launched in 2005 and captured a 10% share of new prescriptions for oral bisphosphonates in the US market. Respiratory GSK continues to be the global leader in respiratory pharmaceuticals with sales of its three key products, Seretide Advair, Flixotide Flovent and Serevent, amounting to 4.0 billion, up 15%. Seretide Advair sales rose 26% to 1.7 billion in the USA. Sales were also strong in both European and International markets, which were up 16% to 1 billion and 0.3 billion, respectively. Central nervous system CNS ; CNS sales declined 8% to 3.2 billion. Sales declined in the USA and Europe, with a small gain in International. Total Paxil sales fell 42% to 615 million, due to generic competition and the interruption in supply to Paxil CR during the year. See `Product supply' on page 49. Partially mitigating this decline was the strong performance of Paxil in Japan, up 17% to 197 million.
Continued consolidation in the pharmaceutical industry could adversely affect the Group's competitive position, while continued consolidation among the Group's customers may increase pricing pressures. The Group had eight products with over 600 million $1 billion ; in annual global sales in 2003. Among these products arePaxil SeroxatandAugmentin, with respect to which the Group now faces generic competition, andWellbutrin SR, Zofran, ImitrexandAvandia, with respect to which the Group is currently defending its intellectual property rights in the USA. If these or any of the Group's other major products were to become subject to a problem such as loss of patent protection, unexpected side effects, regulatory proceedings, publicity affecting doctor or patient confidence or pressure from competitive products, or if a new, more effective treatment should be introduced, the impact on the Group's revenues and operating results could be significant. In particular, the Group faces intense competition from manufacturers of generic pharmaceutical products in all of its major markets. Generic products often enter the market upon expiration of patents or data exclusivity periods for the Group's products. Introduction of generic products typically leads to a dramatic loss of sales and reduces the Group's revenues and margins for its proprietary products. The expiration dates for patents for the Group's major products are set out on page 24. Governmental and payer controls Pharmaceutical products are subject to price controls or pressures and other restrictions in many markets, including Japan, Germany, France and Italy. Some governments intervene directly in setting prices. In addition, in some markets major purchasers of pharmaceutical products whether governmental agencies or private health care providers ; have the economic power to exert substantial pressure on prices or the terms of access to formularies. The Group cannot predict whether existing controls will increase or new controls will be introduced that will reduce the Group's margins or affect adversely its ability to introduce new products profitably. For example, in the USA, where the Group has its highest margins and most sales for any country, pricing pressures could significantly increase upon implementation of the pharmaceutical benefit under Medicare, or in the event that state programmes to control the cost of pharmaceuticals, are adopted. Once the Medicare programme initiates outpatient pharmaceutical coverage for its beneficiaries, the US government, or the private insurers which will offer coverage, through their enormous purchasing power under the programme, could demand discounts that may implicitly create price controls on prescription drugs. Additionally, a number of states have proposed or implemented various schemes to control prices for their own senior citizens' drug programmes, including importation from other countries and bulk purchasing of drugs. The growth in the number of patients covered through large managed care institutions in the USA, which would be likely to increase with implementation of the Medicare amendments, also increases pricing pressures on the Group's products. These trends may adversely affect the Group's revenues and margins from sales in the USA. Until the terms of implementation of the Medicare pharmaceutical benefit have been finalised, it is not possible to quantify the impact of that benefit on the Group's financial results and buspar.

Respiratory GlaxoSmithKline's respiratory franchise is driven by the growth of Seretide Advair, gaining patients from competitor products and the cannibalisation of Serevent and Flixotide. Ventolin and Becotide have faced generic competition for some years but have maintained significant sales. Major respiratory competitors are Singulair from Merck, especially in the USA, Symbicort from AstraZeneca, primarily in the European Union and Spiriva from Pfizer Boehringer Ingelheim, primarily in Europe. Anti-virals The major competitors in the HIV market are Bristol Myers Squibb, Merck and Pfizer amongst others. GlaxoSmithKline has a pioneering role in the HIV market, with Retrovir and Epivir acting as the cornerstone of combination therapy, and available as Combivir in a single tablet. The launches of Ziagen, Agenerase and Trizivir have broadened the Group's portfolio of HIV products. Valtrex has helped strengthen the Group's position in the anti-herpes area, although Zovirax faces competition from generic aciclovir. Both Valtrex and Zovirax compete with Novartis' Famvir. Zeffix was the first anti-viral on the market to treat Hepatitis B. Gilead's Hepsera is the second and was approved by the US Food and Drug Administration FDA ; in September 2002. Anti-bacterials and anti-malarials In 2002 generic versions of both Augmentin and Ceftin Zinnat were introduced in the USA, following successful legal challenges by generic manufacturers see Note 30 to the Financial statements, `Legal proceedings' ; . Augmentin has already lost patent protection in various countries in Europe. The recently launched Augmentin XR, and Augmentin ES compete against a broad range of other branded and generic antibiotics. Malarone's safety profile and convenient dosing regimen have helped put this product in a strong position versus mefloquine following its recent launch for malaria prophylaxis. Metabolic and gastro-intestinal The major competitor for Avanddia is Takeda Chemical's Actos, which is co-promoted with Eli Lilly in the USA. Vaccines GlaxoSmithKline's major competitors in the vaccine market include Aventis Pasteur AP ; , Merck and Wyeth. Engerix-B and Havrix compete with vaccines produced by AP and Merck Comvax and Recombivax HB for hepatitis B, and Vaqta and Avaxim for hepatitis A. Infanrix's major competitor is AP's range of DTPa-based combination vaccines and cardizem.

The National Institute for Health and Clinical Excellence NICE ; is the independent organisation responsible for providing national guidance on the promotion of good health and the prevention and treatment of ill health. Part of its work involves developing clinical guidelines. Clinical guidelines are recommendations, based on the best available evidence, on the most appropriate treatment and care of people with specific diseases and conditions. The implementation of NICE guidance within CNWL is led by the NICE Implementation Committee. This is chaired by Alex Lewis, the Medical Director, and includes Consultant Psychiatrists from each service area across the Trust. The NICE Implementation Committee has recently looked at the NICE guidance on obsessive compulsive disorder OCD ; and post traumatic stress disorder PTSD ; and considered what it means for CNWL. Successful implementation of NICE guidance depends on the activities of all health care professionals. The easiest way to find out about the latest guidance and CNWL's implementation plans is to have a look at the NICE pages on Trustnet. Originally, Plaintiffs moved for a preliminary injunction "restraining defendants, their employees, agents, and successors, and all others acting in concert or participation with them, from enforcing the provisions of H.B. 126." See doc. #2 at 1 ; . Plaintiffs challenged the Act on the following grounds: "the Act is unconstitutionally vague; the Act violates their patients' right to bodily integrity by compelling surgery in circumstances where a medical abortion [via mifepristone, and as opposed to surgical abortion] would otherwise be the desired or appropriate treatment; the Act lacks the constitutionally-mandated exception to allow otherwise restricted.
The observations emphasize the importance of strict hygienic routines and continuous bacteriologic surveillance in maternity departments, as well as close contact between maternity departments and health centers for children.
