DEVELOPMENT AND IMPLEMENTATION OF AN ARGATROBAN TREATMENT PROTOCOL Stephanie N. Argatroban, a direct thrombin inhibitor, is used for systemic anti-coagulation in this setting, however its use is not without challenge; dosage adjustments and monitoring are not clearly defined. The purpose of this study is to develop, implement, and evaluate the effectiveness of an argatroban treatment protocol. An analysis performed on previously collected data at our institution suggested a high incidence of bleeding and little consistency in initial argatroban infusion rates. Rates of administration often were lower at end of infusion than at start of infusion, suggesting that initial doses may need to be lower than indicated in the manufacturer's product information. Frequently, therapy was discontinued due to elevations in aPTT and INR. Based on this data, a treatment protocol was developed; it will be trialed in thirty consecutive patients receiving argatroban. Safety and effectiveness of the protocol will be monitored throughout each patient's course of therapy. Outcomes will be evaluated globally and will include time to therapeutic range, time within therapeutic range, incidence of major and minor bleeding, degree of INR elevation, treatment for elevated INR, and incidence of new thrombosis. Data collection and analysis are ongoing. Upon completion, the effectiveness of the argatroban treatment protocol will be assessed. Recommendations will be made regarding initial infusion rates, dose titration, monitoring, and response to elevated INR. Learning Objectives: Identify the need for an argatroban treatment protocol. Given that amantadine inhibits both M2 and p7 in vitro, we decided to test p7 in a cellbased assay designed to measure M2 function. We found that p7 was indeed functional in this assay and that its activity was inhibited by amantadine. In addition, the p7 of BVDV was also able to substitute for M2 in this assay indicative of ion channel activity. As we have previously shown for HCV p7 Griffin et al., 2003 ; , the BVDV p7 protein was able to oligomerise both in vitro and in cells, further strengthening the case for these proteins being functional and structural homologues. Mutation of the conserved cytosolic loop located between the trans-membrane alpha helices of HCV p7 abrogated ion channel function in this system. Co-expression of this mutant with wild type HCV p7 inhibited wild type activity. No defect in expression was caused by the mutation and both proteins displayed a similar intracellular localisation. Co-localisation of p7 with fluorescent markers for cellular organelles indicated that it largely accumulated in membranes associated with mitochondria. This association was examined longitudinally, in 4, 219 women participants in the Mater-University study of Pregnancy, Brisbane, Australia, over a 14-year period. The bi-directional relationships were both consistent and strong. Overall, negative changes in either direction predicted poor outcomes. Between the post-delivery and 6-months follow-ups, a negative change in relationship quality accounted for 33% of the depression experienced. A negative change in depression accounted for 19% of poor relationships. Negative changes in relationship quality have a greater relative public health impact on mental health of the women in this study than changes in mental health have on relationship quality. 11. The patient's MAR TAR is initialed by the person administering a medication, in the space provided under the date, and on the line for that specific medication dose administration. For patients not in their rooms or otherwise unavailable to receive medication on the pass, the MAR TAR is "flagged" according to facility policy. During routine administration of medications, the medication cart is brought to the doorway of the patient's room with open drawers facing inward and all other sides closed. No medications are left unattended on top of the cart. The cart must be locked if not clearly visible and under the control of the personnel administering medications. An adequate supply of disposable containers e.g., souffl cups ; is maintained on the medication cart for the administration of medications. Disposable containers are never reused. 18. Prior to administration, the medication and dosage schedule on the MAR TAR is compared with the medication label. Information on the medication label should be checked against the MAR TAR at least three times during the med preparation and administration process. If the label and MAR TAR are different and the container is not flagged indicating a change in directions or if there is ant other reason to question the dosage or directions, the physician's orders are checked for the correct dosage schedule. If the medication is discontinued, outdated, or unusable, remove the medication for proper disposal and chlorthalidone.
